Welcome to Our Lady of Tepeyac

Illinois law requires that children must be 4 years old by September 1 in order to enroll in pre-school and 5 years old by September 1 in order to enroll in Kindergarten.
In order to enroll you need the following documents:

Birth Certificate

Baptismal Certificate (if baptized)

Dental Exam: For students entering Pre-Kindergarten, 2nd, and 6th grades. Forms available in the office.

Vision Screening: Students entering our school for the time must bring proof of vision screening. Especially students entering Kindergarten.

Physical and Immunizations: Illinois requires students to have a complete physical and be immunized against measles, mumps, polio, rubella, whooping cough, tetanus, diphtheria, hepatitis B, haemophilus, influenza, and varicella. Especially children entering Pre-Kinder, 2nd, and 6th grades. Forms available in the office.

Academic Records: If possible, please bring most recent report card, withdrawal form from previous school and/or transcripts. Students in grades 6th to 8th will also need copies of any discipline records.

La ley de Illinois requiere que los niños tengan 4 años de edad antes del 1 de septiembre para inscribirse en preescolar (Pre-K) y 5 años de edad antes del 1 de septiembre para inscribirse en kindergarten.
Para inscribir a su hijo/a se necesitarán los siguientes documentos:

Examen de la Vista: Los estudiantes entrando a nuestra escuela por primera vez deben traer prueba del examen de la vista. Especialmente los estudiantes entrando a preescolar.

Examen Físico y Vacunas: Illinois requiere que los estudiantes completen un examen físico y que estén vacunados contra sarampión, paperas, polio, rubéola, tos ferina, tétano, difteria, hepatitis B, hemofilia, influenza y varicela. Especialmente los niños que entran a Preescolar, segundo y sexto grados. Los formularios están disponibles en la oficina.

Expedientes académicos: Si es posible, traiga la boleta de calificaciones más reciente, el formulario de retiro de la escuela anterior y / o expedientes académicos. Los estudiantes de sexto a octavo grado también necesitarán copias de cualquier expediente de disciplina.
